Overview: Bladder tuberculosis, also known as tuberculosis of the bladder, is a chronic, progressive, and destructive condition caused by infection with Mycobacterium tuberculosis. It often occurs as a secondary infection following renal tuberculosis, and in rare cases, it can spread from prostatic tuberculosis. Initial symptoms include urinary frequency, which gradually worsens along with urgency, dysuria, and hematuria. Systemic symptoms such as low-grade fever and night sweats may also be present. Clinically, treatment primarily involves adequate and prolonged administration of anti-tuberculosis drugs, with surgical intervention if necessary.   Epidemiology: Infectiousness: Bladder tuberculosis is considered non-infectious in most cases, as it is an extrapulmonary form of tuberculosis. However, if a patient has concurrent pulmonary tuberculosis, transmission can occur through respiratory routes.   High-Risk Groups: Young adults with a history of pulmonary or renal tuberculosis.   Immunocompromised individuals. Trends in Incidence: While authoritative data on the incidence trend of bladder tuberculosis is lacking, it is worth noting that overall tuberculosis rates have decreased due to improved living standards, environmental awareness, and vaccine promotion. However, recent increases in drug-resistant Mycobacterium tuberculosis, population mobility, and HIV prevalence have led to a resurgence of global tuberculosis cases.   Basic Etiology: Pathogen: Bladder tuberculosis results from invasion by Mycobacterium tuberculosis, the causative agent of tuberculosis. These slender, slightly curved acid-fast bacilli are obligate aerobes with an optimal growth temperature of 37°C. Although they can affect various organs, pulmonary tuberculosis remains the most common form. Route of Infection: Mycobacterium tuberculosis enters the bladder via the respiratory tract and subsequently disseminates through the bloodstream to the kidneys, eventually reaching the bladder via urine flow.   Risk Factors: Poor living conditions and economic disadvantage increase the risk of infection. Individuals with underlying conditions such as diabetes, silicosis, or whooping cough are at higher risk. Immunosuppressed individuals or those receiving steroid or immunosuppressive therapy are also more susceptible to infection.   Typical Symptoms: Bladder Irritation Symptoms: Initial symptoms include urinary frequency, urgency, and dysuria. Nocturia increases gradually, with nighttime voiding occurring 3–5 times initially and later escalating to 10–20 times. Severe bladder mucosal damage may cause burning or painful sensations during urination.   Hematuria: Hematuria is usually microscopic or occasionally visible as gross hematuria with clots. Bladder contraction during voiding leads to ulceration and bleeding. Terminal hematuria is common.   Pyuria: Severe cases may exhibit urine containing cheesy material, appearing turbid like rice soup. Occasionally, blood-tinged or purulent urine may be present.   Systemic Symptoms: Patients with active systemic tuberculosis may experience fatigue, low-grade fever, and night sweats.   Complications: Bladder Contraction: Tuberculous bladder contraction results from severe fibrosis due to involvement of the bladder muscle layer.   Hydronephrosis: Tuberculosis-related bladder narrowing and contraction can obstruct urine flow, leading to hydronephrosis.   Spontaneous Rupture of Tuberculous Bladder: A late-stage complication characterized by full-thickness bladder wall involvement, resulting in caseous necrosis and thinning of affected tissue. Patients may experience sudden abdominal pain without external trauma.   Genitourinary Tuberculosis: Mycobacterium tuberculosis can enter the male reproductive system via the prostatic ducts, ejaculatory ducts, causing prostatitis, seminal vesiculitis, epididymitis, and orchitis.   Diagnostic Workup: Laboratory Tests: Urinalysis reveals acidic urine, positive urine protein, and increased red and white blood cells. Acid-fast staining of urine sediment detects mycobacteria in approximately 50%–70% of cases. Urine culture for tuberculous bacilli (takes 4–8 weeks) is crucial for diagnosing renal tuberculosis.   Tuberculin Skin Test: Evaluates type IV hypersensitivity response to tuberculin antigen, indicating exposure to tuberculosis or response to BCG vaccination. Urinary Tract Plain Film + Intravenous Urography (IVU): Urinary Tract Plain Film (UTPF): UTPF is useful for observing bladder calcifications. Intravenous Urography (IVU): IVU provides information about renal function, the extent of lesions, and the affected areas.   MRI: MRI with water imaging is particularly valuable for diagnosing concurrent renal tuberculosis and assessing the presence of hydronephrosis. Other Examinations:   Cystoscopy: A specialized procedure involving the insertion of an instrument with a light source through the urethra to visualize changes within the bladder. Cystoscopy allows observation of bladder mucosal lesions, measurement of bladder volume, and collection of clean urine samples for further examination. Overview of Bladder Cancer   Bladder cancer refers to a malignant tumor that occurs on the bladder mucosa. It is the most common malignant tumor in the urinary system and one of the top ten common tumors in the body. It accounts for the first place in the incidence of urogenital tumors in China, and its incidence in the West is second only to prostate cancer, ranking second. In 2012, the incidence of bladder cancer in the national cancer registration area was 6.61/100,000, ranking ninth in the incidence of malignant tumors. Bladder cancer can occur at any age, even in children. Its incidence increases with age, and the high incidence age is 50-70 years old. The incidence of bladder cancer in men is 3-4 times that of women. In the past, the epithelium on the bladder mucosa was called transitional cells. In 1998, WHO and the International Urological Pathology Society jointly recommended the use of the term urothelial instead of transitional cells to distinguish it from transitional epithelium in the nasal cavity and ovaries, making urothelial a proprietary term for the urinary system. In the 2004 WHO “Pathology and Genetics of Urinary System and Male Reproductive Organs”, the histological classification of urinary system tumors includes bladder urothelial carcinoma, bladder squamous cell carcinoma, bladder adenocarcinoma, and other rare ones include bladder clear cell carcinoma, bladder small cell carcinoma, bladder-like carcinoma. The most common is bladder urothelial carcinoma, which accounts for more than 90% of the total number of bladder cancer patients. Usually, bladder cancer refers to bladder urothelial carcinoma, which was previously called bladder transitional cell carcinoma. Causes of Bladder Cancer   The cause of bladder cancer is complex, with both inherent genetic factors and external environmental factors. The two most clear pathogenic risk factors are smoking and occupational exposure to aromatic amines. Smoking is currently the most certain risk factor for bladder cancer. 30%-50% of bladder cancer is caused by smoking. Smoking can increase the risk of bladder cancer by 2-6 times. As the smoking time prolongs, the incidence of bladder cancer also significantly increases. Another important pathogenic risk factor is related to a series of occupations or occupational exposures. It has been confirmed that aniline, diamino diphenyl, 2-naphthylamine, 1-naphthylamine are all carcinogens of bladder cancer. Long-term exposure to these chemicals increases the probability of bladder cancer. Occupational factors account for about 25% of the total number of bladder cancer patients. Occupations related to bladder cancer include aluminum products, coal tar, asphalt, dyes, rubber, coal gasification and other industries. Clinical Manifestations   About 90% or more of bladder cancer patients initially present with hematuria, usually manifested as painless, intermittent, gross hematuria throughout the course, sometimes also as microscopic hematuria. Hematuria may only appear once or last for one day to several days, and it can alleviate or stop on its own. Sometimes the coincidence of the patient taking medicine and the cessation of hematuria often gives the patient the illusion of “cure”. Some patients may have hematuria again after a certain period of time. The color of hematuria varies from light red to dark brown, often dark red. Some patients describe it as meat washing water and tea water. The amount of bleeding and the length of hematuria are not necessarily proportional to the malignancy, size, range, and number of tumors. Sometimes when gross hematuria occurs, the tumor is already large or late; sometimes a small tumor can cause a large amount of hematuria. Some patients are found to have tumors in the bladder during health check-ups by B-ultrasound. 10% of bladder cancer patients may first present with bladder irritation symptoms, manifested as frequent urination, urgency, dysuria, and difficulty urinating, and the patient has no obvious gross hematuria. This is mostly due to tumor necrosis, ulceration, large or multiple tumors in the bladder, or diffuse infiltration of bladder tumors into the bladder wall, causing a decrease in bladder capacity or concurrent infection. Tumors in the bladder trigone and bladder neck can obstruct the bladder outlet and cause symptoms of difficulty urinating.   Clinical Diagnosis   Physical examination and past history Examine the body to determine whether the general signs of health are normal, including checking for disease markers, such as lumps or anything else that looks abnormal. The patient’s health habits, past diseases and treatment history are also part of the examination. Internal examination For vaginal and/or rectal examination. The doctor will insert a gloved finger into the vagina and/or rectum to feel for any lumps. Urinalysis Check the color of the urine and the substances in the urine, such as sugar, protein, red blood cells, and white blood cells. Urine cytology examination Check the urine under a microscope to see if there are abnormal cells. Cystoscopy Cystoscopy is inserted into the bladder through the urethra. The cystoscope is a thin tubular instrument with a light source and a mirror for observation. The cystoscope may also have tools for tissue sampling, and the tissue samples are then examined under a microscope for signs of cancer.   Intravenous pyelography (IVP) A series of X-ray examinations of the kidneys, ureters, and bladder to see if cancer is present in these organs. The contrast agent is injected into the vein, and when the contrast agent moves through the kidneys, ureters, and bladder, an X-ray examination is performed to see if there are any blockages. Biopsy Remove part of the cells or tissues and have a pathologist look at them under a microscope to check for signs of cancer. The biopsy of bladder cancer is usually done during cystoscopy, and the entire tumor may be removed during the biopsy. Clinical Suggestions   Surgical treatment (1) Transurethral resection (TUR) with electrocautery: A surgery that inserts a cystoscope (thin and light tube) into the bladder through the urethra. Then use a tool with a small wire loop to remove the tumor or use high-energy electric shock to burn the tumor. This is what is called electrocautery. (2) Partial cystectomy: A surgery to remove part of the bladder. This surgery is suitable for patients with low-grade cancer, and the tumor has invaded the bladder wall but is limited to part of the bladder. Because only part of the bladder is removed, the patient can urinate normally after the surgery. This is also called segmental cystectomy. (3) Radical cystectomy: A surgery to remove the bladder and any lymph nodes and nearby organs containing cancer cells. When bladder cancer invades the muscle wall, or when superficial cancer affects a large part of the bladder, this surgery can be performed. For men, the nearby organs to be removed are the prostate and seminal vesicles. For women, the uterus, ovaries, and part of the vagina will be removed. Sometimes, when the tumor has spread outside the bladder and cannot be completely removed, a surgery to remove only the bladder can be performed to reduce the urinary symptoms caused by cancer. When the bladder must be removed, the surgeon will create another way to urinate. (4) Urinary diversion: A surgery to create a new way for the body to store and excrete urine. Even if the doctor removes all the cancer cells visible during the surgery, there will still be patients who will undergo chemotherapy after the surgery to kill any possible remaining cancer cells. The treatment given after the surgery to reduce the risk of cancer recurrence is called adjuvant therapy. Radiation therapy Radiation therapy is a type of cancer treatment that uses high-energy X-rays or other types of radiation to kill cancer cells or prevent them from growing. There are currently two types of radiation therapy. External radiation therapy uses a machine outside the body to radiate cancer. Internal radiation therapy uses radioactive substances sealed in needles, implanted particles, wires, or catheters, directly placed in and acting on cancer or near cancer. The way radiation therapy is given depends on the type and stage of the cancer being treated.   Chemotherapy Chemotherapy is a method of using drugs to stop the growth of cancer cells, either by killing the cells or by preventing the cells from dividing. When chemotherapy is given by mouth or by injection into a vein or muscle, the drug enters the bloodstream and reaches all the cancer cells in the body (systemic chemotherapy). When chemotherapy drugs are placed directly in the cerebrospinal fluid, an organ, or a body cavity such as the abdomen, the drugs will mainly affect the cancer cells in these areas (regional chemotherapy). For bladder cancer, regional chemotherapy will be performed inside the bladder (put into the bladder through a tube inserted into the urethra).   Biological therapy Biological therapy is a treatment method that uses the patient’s immune system to fight cancer. Use substances produced by the body or in the laboratory to promote, induce, or restore the body’s natural defense mechanism against cancer. This type of cancer treatment is also called biotherapy or immunotherapy. Treatment of bladder cancer can also use BCG (Bacillus Calmette-Guerin) intravesical biological therapy. Use a catheter (thin tube) to directly put the solution containing BCG into the bladder.   Living Care for Bladder Cancer   Psychological Care Patients with bladder cancer are prone to anxiety and panic, and they may struggle to accept the reality of urinary diversion. Healthcare providers should establish a good relationship with patients, actively communicate with patients and their families, and fully earn the trust of patients. They should explain the relevant knowledge of this disease in detail, inform patients of possible complications and adverse reactions, eliminate patients’ fear, anxiety, and even despair, and encourage patients to face the disease with a positive attitude and actively cooperate with treatment.   Disease Observation Before surgery, it is important to closely observe the patient’s hematuria. After surgery, closely monitor the patient’s vital signs, continuously monitor electrocardiography, observe and record changes in the patient’s pulse, heart rate, blood pressure, etc., and ensure the smoothness of the patient’s blood transfusion and fluid infusion. Observe the wound exudation situation and change dressings in time. Regularly monitor changes in the patient’s white blood cells and body temperature, and observe whether an infection occurs. Observe the color, nature, and volume of various drainage fluids. If a large amount of bright red fluid is drained in a short period of time, the doctor should be notified in time for treatment.   Dietary Care Before surgery, patients should be given high-protein, easily digestible, and highly nutritious food. For patients with difficulty eating, intravenous nutritional support should be provided to correct anemia and improve the patient’s overall nutritional status. After transurethral bladder tumor electroresection surgery, the patient can eat 6 hours later. The diet should be rich in nutrition and coarse fiber foods, and spicy and irritating foods should be avoided.   Drainage Tube Care Different labels should be attached to various drainage tubes for distinction. The drainage tube should be properly fixed to prevent it from falling out. Keep the drainage smooth and avoid bending. Observe and record changes in the nature, color, and volume of the drainage fluid.   Pre- and Post-Chemotherapy Care If the patient’s condition allows and there is an indication for radiotherapy and chemotherapy, radiotherapy and chemotherapy will be performed half a month after surgery. Patients who can hold urine after bladder preservation surgery will immediately undergo bladder perfusion chemotherapy. Before bladder perfusion, the urine should be emptied to prevent dilution of the chemotherapy drug. Gallbladder removal surgery, also known as a cholecystectomy, is a common procedure to remove the gallbladder, a small, pouch-like organ located in the upper right part of the abdomen that stores bile. Bile is a fluid produced by the liver that helps break down fatty foods. The gallbladder is not an essential organ, so surgery to remove it is often recommended if problems develop.   The most common reason for gallbladder removal is the presence of painful gallstones. These are small stones that can form in the gallbladder as a result of an imbalance in the substances that make up bile. Gallstones can cause no symptoms, but occasionally they can block the flow of bile and irritate the gallbladder or pancreas, causing symptoms such as sudden and intense abdominal pain, nausea and vomiting, and jaundice. Surgery to remove the gallbladder is the most effective treatment in most cases.   There are two main ways to remove a gallbladder: laparoscopic (keyhole) cholecystectomy and open cholecystectomy. In a laparoscopic cholecystectomy, several small incisions are made in the abdomen and fine surgical instruments are used to access and remove the gallbladder. In an open cholecystectomy, a single larger incision is made in the abdomen to access and remove the gallbladder. Keyhole surgery is used most often because it allows for a faster recovery and smaller scars.   The typical steps of a laparoscopic cholecystectomy are as follows: 1. Creation of several small incisions on the abdomen. 2. Inflation of the abdomen for easy access. 3. Insertion of the laparoscope (small camera) and tiny surgical instruments through the incision. 4. Separation of muscles around the surrounding liver to expose the gallbladder. 5. Dissection and extraction of the gallbladder. 6. Closure of incisions.   Recovery from keyhole surgery to remove the gallbladder usually does not take long. Most people can leave the hospital on the same day or the next morning and return to most normal activities within two weeks. Recovery from open surgery takes longer; patients may need to stay in the hospital for three to five days and it could be six to eight weeks before they feel back to normal.   After gallbladder removal, patients can lead a perfectly normal life without a gallbladder. The liver will still produce enough bile to digest food, but instead of being stored in the gallbladder, it will drip continuously into the digestive system. Some people may experience bloating or diarrhea after surgery, but this usually improves within a few weeks. If certain foods or drinks trigger these symptoms, patients may wish to avoid them in the future.   In summary, gallbladder removal surgery is a common and low-risk procedure used to treat problems associated with the gallbladder, such as painful gallstones. The surgery can be performed using keyhole or open techniques and recovery time varies depending on the method used. After surgery, patients can lead normal lives without a gallbladder.

How to Use a Disposable Balloon Trocar | SurgSci
A disposable balloon trocar is a medical device used in laparoscopic surgery. It has a blunt tip and an inflatable balloon at the end which is inflated after insertion to create a seal around the incision site and prevent gas leakage during surgery. The device is used to create a port of entry for other surgical instruments during laparoscopic surgery.   Here are the general steps to use a disposable balloon trocar: 1. Make an incision in the skin using a scalpel. The incision should be small enough to allow the trocar to pass through but large enough to accommodate the surgical instrument. 2. Insert the trocar through the incision. The trocar should be inserted at a 90-degree angle to the skin and advanced until it reaches the abdominal cavity. 3. Inflate the balloon at the end of the trocar using a syringe. The balloon should be inflated with sterile saline solution or carbon dioxide gas until it creates a seal around the incision site. 4. 1. Take the obturator and cannula out of the box using sterile technique. 2. Create an incision and ensure the abdominal cavity has been entered (refer to Hasson abdominal entry skill). 3. Pass two sutures through each fascial edge, assemble the obturator and cannula, insert the trocar into the cavity with suture lie post secured on the cannula. 4. Wrap the suture line around each suture tie post to make the cannula stable on the abdomen, unlock the cam on suture tie post to adjust cannula position in abdominal cavity. 5. Lock the suture tie post, press both locking buttons on the obturator top and release the obturator from cannula. 6. Connect the gas line and open the stopcock to make pneumoperitoneum, then insert surgical instruments through cannula for operation. 7. Push the outer seal release lever on the cannula to release the outer seal for specimen removal procedure during surgery. 8. Recently, the biomedical materials R&D team of Shenzhen SurgSci Medical Ltd. made a research achievement titled "Hyaluronic acid and chitosan-based injectable and self-healing hydrogel with inherent antibacterial and antioxidant bioactivities" was published in the international SCI academic journal "INTERNATIONAL JOURNAL OF BIOLOGICAL MACROMOLECULES" (Chinese Academy of Sciences SCI Journal Area 1, IF=8.025, TOP journal).   Based on natural biomacromolecules, quaternary ammonium chitosan (QCS), tannic acid (TA) and oxidant hyaluronic acid (OHA), a new product called QCS-l-TA-l-OHA was developed. The hydrogel dressing ("l" is "link"), whose research inspiration comes from natural substances such as quaternary ammonium chitosan (QCS), tannic acid (TA), oxidant hyaluronic acid (OHA) and their derivatives The antibacterial, antioxidative and wound healing functions of the substance have fully exerted the biological activity of the new intelligent multifunctional hydrogel, a natural substance and its derivatives, and has broad application prospects for the development of the biomedical field.     In order to study the intrinsic antibacterial and antioxidant biological activities of the dressing, the team members designed the molecular structure of the QCS-l-TA-l-OHA hydrogel and used the hydrogel as a rat full-thickness skin incision model wound. Compared with radical resection of rectal tumors, local resection of rectal tumors has the advantages of small surgical trauma, low risk, fast postoperative recovery and short hospital stay, in particular, it is necessary to avoid artificial anus dysfunction and obstacles caused by radical surgery and postoperative voiding function and sexual function.   There are many surgical methods and methods for local excision of rectal tumors, such as transanal approach, transsacral approach, transsphincter approach, and transanal endoscopic approach. They overlap and differ in surgical indications.Due to the different surgical approaches and methods, postoperative complications also vary. Therefore, familiarity with these surgical methods, and on the premise of strictly grasping the surgical indications, correct selection of surgical methods can significantly improve surgical efficacy and reduce surgical complications.   At present, there are various approaches and methods for local excision of early rectal tumors. The following is a brief introduction to various surgical methods for local excision of rectal tumors. 1. Transanal local excision of rectal tumor This is one of the most ancient and common surgical methods. Its advantages are direct surgical approach, simple operation, no surgical incision on the body surface after operation, and less trauma and surgical risk. But its disadvantages are: (1) Due to the position of the patient during the operation, most doctors can only perform surgery in a sitting position, and the surgeon's line of sight and the lesion are in the horizontal position instead of the traditional overlooking surgery that conforms to the surgeon's habits. (2) The degree of freedom of operation of surgical instruments in the rectum and anal canal is limited, and can only be performed within 45 degrees at most, and many fine operations cannot be performed. As a result, it is difficult to provide meaningful guidance for postoperative treatment of patients.   All of these may ultimately bring some disadvantages and uncertainties to the future surgical efficacy. Surgical indications for transanal resection are therefore usually limited to: ž The lesion is located within 4-5cm from the anal verge. ž The diameter of the lesion is not greater than 2cm. ž If it is a malignant tumor, it must be Tis-T1N0. The more common complications after this operation are postoperative wound bleeding and urinary retention. Accurate mastery of suturing techniques and preoperative prophylactic placement of urinary catheters can prevent the occurrence of these complications.   2. Local resection of rectal tumor through the sacrococcygeal region Transsacral rectal tumor resection is also known as Kraske's operation; the procedure was pioneered by German surgeon Kraske in 1885. The surgical steps are briefly described as follows: The patient was placed in a prone position, and a straight incision was made from the sacrococcygeal joint to the anus. After the coccyx was removed, the pelvic floor muscles were separated to expose the posterior rectal wall, and the posterior rectal wall was incised at the upper edge of the puborectalis to expose the tumor in the rectal cavity. Instruction for Use-Disposable Endo Cutter Stapler and Cartridge 1. Remove the device from the package. Choose the suitable sized cartridge. Insert the cartridge to the cartridge cave. Ensure to align the arrows on both sides to have the cartridge loaded successfully. 2. Before firing, please make sure the cartridge is properly loaded. Clench the active handle the cartridge is closed. Then pull the release button to the starting point. If the cartridge is open, this indicates the cartridge is properly loaded. Clench the trigger lightly again to make the cartridge closed. 3. Insert the endoscopic linear cutter into the trocar sleeve until the articulation of the cartridge is visible. Pull the release button back to the starting point the cartridge is open. (This step is not shown in the video) 4. Adjust the device according to the target tissue. Turn the articulation lever lightly and the cartridge head will move at the same direction. 5. Clench the trigger and release. Repeat this action until the trigger cannot push. Check if the black button reaches the schedule stapling position. After firing, keep at least 20 seconds in order to staunching bleeding. 6. Move the device carefully. Turn the reset joint to the 0 position. Clence the trigger to close the jaw. Pull the release button to the starting point to open the jaw. Pull the black button downward to the end. Rotate the cartridge 45 degrees anticlockwise. Remove the cartridge of the device. Check the staple line, the process completes.

1. Using sterile technique, remove the device from the package. 2. Select the appropriate size ligating clip cartridge and corresponding clip applier. 3. Grasp the applier by the handle and trigger. 4. Insert the jaws of the instrument into the individual cartridge slot, making sure the tips of the applier are perpendicular to the surface of the cartridge. Insert the applier until it stops. Do not force the applier. It should enter and withdraw from the cartridge smoothly. 5. Keeping the applier jaws perpendicular to the surface of the cartridge, retract the applier from the cartridge. The clip will be securely helped in the applier jaws. It is not necessary to maintain trigger tension to hold the clip in the applier jaws. 6. Position the clip around the tubular structure to be ligated. Apply sufficient force to fully close the applier to assure that the clip is satisfactorily placed and secured.
